What Kind of Physical Therapy is Recommended After a Metal on Metal Hip Replacement?

After your metal-on-metal hip replacement surgery, you will feel very tired and aware of your surgery site. You may feel tempted to remain in bed, but it’s actually very important for you to resume physical activity right away. Of course, you need to be careful not to open the incision or interfere with the healing required. That’s why a physical therapist will instruct you regarding both in hospital and out of hospital recovery exercise.

The First 24 Hours
The physical therapist will have you up and taking a few steps within 24 hours of your surgery. You will also be required to sit up in bed and allow your feet to dangle over the side of the bed.

As you lie in bed on your back, you can perform these three exercises, each of which will help prevent blood clots and improve circulation.

Ankle Rotations
Tilt your foot such that you point the foot inward towards your other leg, and then rotate your ankle such that you point your toe outward, towards the edge of the bed. Repeat whenever you think of it.

Flex and Point
Flex your foot, and then point your toe, moving your foot back and forth flexing and pointing. Repeat this exercise whenever you think of it.

Buttock Clenches
Squeeze your buttocks together and hold for a count of five. Then release and relax. Repeat this exercise whenever you think of it.

The First Couple Days
You will be in the hospital for anywhere from two to four days. After that you will either be transferred to a rehabilitation center or to your home, where an occupational therapist will work with you in addition to the physical therapist.

Your primary goal will be to learn how to walk again. Most likely you will need to use a walker or to hold onto the nurse or therapist. You will graduate from a walker to crutches (about a month after the surgery) and finally to a cane, but that can take quite a while. Your first goal will be to walk using the walker.

You will also be taught how to dress yourself, bathe yourself and perform other everyday tasks while protecting your surgery site.

In addition to these practical physical exercises, you will want to do the following physical therapy exercises:

Knee Raises
Stand up and hold onto the end of your bed for support.
Shift your weight to the leg that is completely well, and carefully lift the foot of your recovering leg off the floor, bending your knee and lifting that knee. Bring the knee up towards your chest but no higher than your waist. Hold as you breathe in and out once, and then set it back down.

Leg Lifts
Stand up and hold onto the end of your bed for support. Shift your weight onto your healthy leg. Lift the other leg, to the side, gently and carefully extending the leg. Return to position and repeat.

Full Recovery
You will need to exercise daily, walking and performing these exercises until you reach full recovery.

You will also probably be prescribed two medications: a blood thinner and pain medicine. Take the blood thinner for a month and the pain medicine for anywhere from a week to two weeks. The combination of the medication and exercise should get you back into tip top shape.
