What is Hip Revision Surgery and How Does it Relate to Metal-on-Metal Hip Replacements?

Hip revision surgery is when a current prosthetic hip implant is removed and a new implant is put in place. It is a complex procedure that requires special tools and implants, surgical know-how and planning.

Sometimes these surgeries require the use of bone grafts. An increasing number of younger patients are receiving hip replacement implants and older patients are living longer. For these reasons patients are more frequently outliving their implant and needing revision surgery.

Why Hip Revision?

There are several reasons why a surgeon may feel that hip revision is the best course of action. The hip joint may become very painful and swollen, possibly due to implant loosening or wear, or even from infection. The implant may also fail to function properly, this typically happens after an extended time in place. Also, a follow-up x-ray may reveal the device no longer in proper position or indicate wear of the components.

Loosening can occur when the implant, intended to be permanent, continues to rub from daily activity, particularly in highly active individuals. This friction can lead to microscopic particles being released from the implant. These particles become embedded in the tissue surrounding the joint, ultimately leading to damage to the tissue and bone and implant loosening.

Dislocation is another problem that can lead to revision. Sometimes this is caused by loosening and the symptoms associated with it but it can also be caused by patients not adhering to physician's orders on posture and movement.

Implants can fail due to infection. Hip replacement surgery involves large prosthesis of metal, plastic or ceramic that can be a place for bacteria to stick to. Changing blood supply to the area during surgery can make it harder to fight infection.

The following types of patients tend to cause faster wear to their original hip implant and more frequently require hip revision surgery:

Metal-on-Metal Implants

Metal-on-metal implants can have an increased amount of hip revision surgeries. One reason is that metal-on-metal is frequently used on very active patients who plan to return to a highly active lifestyle after surgery. This particular type of implant is believed to be more durable for these patients compared to metal and plastic or all ceramic prosthesis.

Another reason metal-on-metal implants see a higher number of revision surgeries is that with the wear and friction that is typical of daily use, over time microscopic fragments of metal are released into the body. These metal ions can get into the soft tissue in the hip are and cause loosening and other painful reactions that require action.
