What Happens Before, During and After a Metal-on-Metal Hip Replacement Surgery?

Are you about to have a metal-on-metal hip replacement? Are you wondering what to expect with the entire process? Before surgery, the physician meets with the patient to review health and lifestyle factors that impact whether or not metal-on-metal hip replacement is the right choice. Some factors the surgeon considers are:

•    Gender
•    Age
•    Weight
•    Activity level
•    Medications taken prior to surgery, particularly corticosteroids
•    Metal sensitivity or allergies
•    Renal function
•    Heart function
•    Immune system function

The surgeon will weigh the risks and benefits of each type of implant compared with the factors listed above and discuss the options with the patient. The physician may require the patient to take antibiotics prior to surgery as well as after. And if any major dental work is needed, may also request it be done before surgery. This is because the risk of infection traveling to the implant area from the mouth is great.

Once the surgeon and patient have determined that metal-on-metal hip replacement is the best choice surgery is scheduled. In this procedure the damaged bones of the hip joint are replaced with metal prosthetics, most likely made of cobalt and chromium, though there is record of these implants containing titanium and stainless steel as well. The metal implants are attached to the remaining bone either with cement or a porous coating that then allows new bone to grow into the pores and adhere the implant to the bone.

Anesthesia used will depend on preference of the surgeon and the patient as well as the patient's overall health. Often general anesthesia is used. This means the patient is fully unconscious during the procedure. Sometimes local anesthesia is preferred. This numbs the area and makes the patient somewhat drowsy, but they are awake.

After Surgery

When you awaken from surgery you may notice some or all of the following:

•    A catheter in place to empty your bladder
•    A compression stocking on your leg to assist proper circulation
•    A cushion keeping your new hip in proper alignment
•    IV antibiotics, most likely for a day post-surgery

You will also receive pain medication and medication to stop blood clots. Due to the decrease in your activity level, your doctor may teach you some breathing exercises as well as ways to move your feet to aid in circulation.

Pain and blood clot medicine will continue for a short time after surgery. The specifics of your surgery determine how soon you can bear weight on the new joint. Once you are cleared for it you will begin rehabilitation for the joint. You will also be taught how to do daily things like cooking, dressing, walking and sitting while allowing your new joint to heal.

Rehabilitation, exercises and care continue for some time after returning home. At times the process may seem slow but allowing the joint the proper time to heal ensures the best outcome over the life of the implant and the best quality of life for the patient.
