Can a Skin Rash be Related to a Metal-on-Metal Hip Replacement?

Total hip replacement surgery replaces the ball and socket joint of the hip with a prosthetic implant. There are several options of materials that can be used. Each option has its own set of risks and benefits as well as factors that make it best suited for certain people.

One such option is metal-on-metal hip replacement. With this option both the ball and socket of the implant are made of metal with no other material used as a spacer. Metals that can be used are titanium, stainless steel and cobalt chrome.

Metal Fragments

Five percent of metal sensitivities are linked to cobalt and chromium. Discussing a patient's metal sensitivity is part of the evaluation processes prior to surgery. Not everyone who has a metal sensitivity is aware of it. Also, people can react to certain levels of metal within the body without being allergic or sensitive.

There is a concern whether these metals can be corroded by just being in contact with bodily materials and how long that takes. When these metal implant pieces rub together during normal joint movements they can release microscopic fragments of metal into the body.


These microscopic fragments traveling through the body can signal an allergic reaction by your body even if no previous metal allergy has been determined because your immune system may perceive the particles as foreign. This allergic reaction can show through pain, swelling, fever, inflammation, eczema and rashes. Besides being uncomfortable, these symptoms can lead to further problems such as skin infections.

Cause for Concern?

Many patients have reported instances of localized or spreading rashes near or around the surgery site. Sometimes these rashes are in fact from laying in certain positions or on certain bedding for extended periods during recovery. Other times the rashes are a reaction to medication.

There are cases where the metal-on-metal implant is believed to be the cause of the rash. This is most likely due to a metal sensitivity either from simply having the implant in place, or from the microscopic fragments entering the system. Any symptoms should be reported to the physician as part of the post surgical care plan in place.

Although there can be some cause for concern, the FDA does not feel it is necessary to set a standard level of metal ions to require review or intervention by physicians. These rashes should be reported to the physician immediately and dealt with individually based on health history, symptoms and lifestyle factors.
