What Causes Hip Replacement Loosening?

Loosening of a hip replacement is one of the main reasons surgery is needed years after the initial placement of an implant. When the joint loosens, it leads to pain and a reduction in the movements a person can make. There are several reasons for loosening, but osteolysis is one of the most common causes. It occurs when the bone and tissue around the implant degrade and are no longer able to hold the implant securely in place.

Typically, hip replacements last about 20 years without any problems. There are exceptions to this rule, but most doctors prefer to place hip implants in patients who are at least in their 60s.This reduces the likelihood a second operation will be needed. Younger patients typically have more years left to live, increasing the odds revision surgery will be needed at some point.

What Causes Osteolysis?

As hip implants wear, small particles break away. These particles and flakes irritate the surrounding tissue. The immune system responds to the flaking with an attack because it is viewed as a foreign invader. In addition to the particles, the immune system also attacks the tissue near the implant, creating holes and weakening the area. This process is known as osteolysis. When the bone and tissue can no longer support the implant, it gradually loosens and becomes painful.

In addition to osteolysis, hip implants can loosen as a result of normal wear and tear on the implant device. Since the artificial joint is exposed to near-constant friction, the breakdown process is faster than it would be for another type of artificial device. Unfortunately, it is also not as strong as a person’s natural hip when it is healthy.

When a hip replacement becomes loose, revision surgery may be needed to fix the problem. Revision surgery is similar to the initial placement of the device, but there are some differences. These include:

•    Greater risk of infection
•    Longer surgery time
•    Need for more blood than during the placement surgery
•    Extensive pre-op procedure
•    Replacement of lost bone through grafting or implant
•    Longer recovery
•    Greater risk of reduced range of motion after recovery

The goal of the medical community is to develop implants that are longer lasting and rarely need to be replaced, but until that happens, revision surgery will be necessary for some patients.
