What Does a Metal-on-Metal Hip Replacement Process Look Like?

If you’re going in for a metal-on-metal hip replacement surgical procedure, you’re probably wondering what the surgeon will be doing while you’re under anesthesia. The following is a detailed description of what you will experience during your metal-on-metal hip replacement procedure.

Before Your Surgery
You will be asked to abstain from food and water the morning of your surgery so that you will not have problems tolerating the anesthesia. Some people, if they eat or drink before surgery, may vomit during or after the surgery since the anesthesia can upset your stomach. It is safest to go into surgery with an empty stomach.

 When you get to the hospital, the intake staff will check your vital signs, ask you pertinent questions and then insert an IV (intravenous tube.) This is how you will receive fluids and any necessary medications, such as anesthesia, antibiotics and pain medications.

You will be given anesthesia and wheeled into the operating room. You will be unconscious during the surgery, so you won’t feel a thing.

During the Surgery
You won’t be aware, but the surgery will last anywhere from one to four hours. While you are unconscious, the operating staff will insert a catheter (in case you need to void your bladder) and compression stockings on your legs.

The surgeon will make an incision in your hip and then will remove the ball-end of your femur (the bone in your thigh that feeds into your hip socket.) The ball end will be replaced with a metal ball and connected rod, called the stem. In some cases the socket itself will also need to be replaced with a metal socket. The implant will be either cemented or grafted in place (using what is called biologic fixation, in which a porous surface of the implant is grafted to bone, where it naturally grows together over time.) The artificial parts will act as though they have always been a part of your body.

The surgeon may shave or otherwise resurface the socket to ensure a perfect fit of the ball into the socket or to ensure proper cementing or grafting. The surgeon will also trim any excess cartilage or bone that is diseased or otherwise damaged and deemed unhealthy. When the fit is determined to be just right, the surgeon will stitch you back up and prepare you for recovery.

When You Wake Up
You will still be under the influence of the anesthesia, so you will feel okay when you first wake up. However, as the anesthesia wears off, you will need painkillers and will become aware of how exhausted you are. The nurse will probably use a pillow or foam support to keep your hip in the right position. You will be taught how to move and position yourself to have the least amount of discomfort possible.

You will have drainage tubes coming out of the incision site, but the nurse will watch these to make sure everything is draining properly and there are no signs of infection. The nurse will have you take your first steps in about 24 hours, and you will be guided through the recovery process by nurses and physical therapists over the following two to four days. You may be in a rehabilitation facility for a while after that, or you may return home, depending on your recovery progress and the level of support and care you have at home.
